Entering the CFA Awards Program is easy. Simply fill out this short form and click on the button below to request the survey. You will receive an e-mail from InfoFeedback within one (1) business day, which will contain a link to the survey and contain more information about the process. Note: If you would like to enter more than one franchise brand, you must complete an individual survey for each brand.

This request form is exclusively for a CFA member franchise system to initiate their entry into the CFA Awards Program. If you are a franchisee of a participating CFA member franchise system and would like to request your unique survey link, please click here.
